Yes, because doctors will want to give it to someone who'll actually take care of it.
It's not to say that those who smoke and drink don't deserve it, but they're given specific instructions not to indulge, and if they can't follow them, they won't follow them when they have their new organ, either.
Not everyone is a donor, so not everyone on a list is going to get what they need. Priority goes to those who'll get the best use of it.
Or who'll pay the most.
