Well usually you go through safety training, so if you completed that training you should know how to avoid these mistakes, but ofc mistakes happen. Depends how you made it happen, if you were smoking on a job when it's forbidden, then yeah that's on you completely. In other cases, I'd say it's up to your employer, whether they decide to keep you or not.
