That's just in the movies. Therapists prefer to sit face-to-face with their patients, but offset a little so they are not directly in front of the patient.

haha, i guess if people want to do that, they have the option to. i've never laid down on a couch in therapy -- i just sit there.

I've never laid down in therapy before. Maybe that's a service only offered to those with more money to be able to afford the lying down.
